Contractor Lien Waivers are to be delivered with all contractor invoices, with exception of service contracts, that do not exceed $3,000.

For invoices or projects in excess of $3,000 SMG requires that a Lien Waiver and Affidavit be completed.  Please select from the Lien Waiver and Affidavits that are supplied below for the document that best fits your Company Structure and payment request.  You may contact the SMG Accounting Department if none of the supplied documents meet your needs.
